3. TO equal access to services regardless of race, religion, sex or handicap

4 TO BE informed about your services involving significant risks

5. THE best services available to meet your individual needs

6. TO make suggestions and participate in the development of your service plan

7. TO Confidentiality

8. TO ask and be told about your rights

9. TO not be subject to research without written and informed consent

10. TO inspect, copy or correct your records

11. TO have a copy of the rules of conduct that apply to services you are receiving

12. TO get help in exercising your rights and file a complaint when you think your rights have been violated; and obtain a fair and objective grievance process

 

If you believe your rights have been violated or have complaints about your services, you are encouraged to:

1. TO discuss the complaint with your primary staff member.

2. TO discuss with that staff member’s supervisor if you are not satisfied.

3. TO request a meeting with the division director if you are still not satisfied.

4. TO request a meeting with the Executive Director if complaints have not been satisfied.
